Most medications will be offered through CCMD. When you fill out a request for membership please list all current medications because we will let you know before accepting you if there are medications we are unable to fill.
CCMD does not bill insurance for your visits as they are covered by your annual fee. We will however bill your insurance through our lab company when doing lab work or pathology.
If you have a urgent or life-threatening emergency, call 911 immediately or go to your nearest Emergency room. If you have a non-urgent problem, please contact me first.
Most can use an HSA to pay for CCMD services. But you should check with your individual HSA to be sure.
We are happy to discuss and set up optional payment plans through CCMD.
You can pay by cash, check or we can invoice you and you will be able to pay by debit or credit card.
CCMD is always available for messaging and/or phone calls during normal business hours (7am- 7pm).

The initial consultation may last from 1-2 hours depending on your needs. Additional office visit lengths are at the discretion of the physician and patient.
If you need specialty care that CCMD cannot provide we will assist with referrals to the appropriate physician. Typically CCMD will directly contact the specialist to discuss your needs and ensure timely follow up. Most lab work is able to be done at CCMD. If you need imaging performed we will arrange that appointment at the imaging center of your choice.
In many cases yes. Please discuss with your accountant.
If you have paid your annual fee in full and wish to cancel we will prorate the amount refunded. After the initial consultation if you decide not enroll you will only pay the initial $250 consultation fee.
Yes. CCMD will not take the place of general health insurance coverage. CCMD is a primary care medical practice, not a health insurance program. You are advised to continue your current coverage.
You are welcome to email and ask any questions you may have prior to scheduling a consultation. When you request a consultation you will fill out a form that asks several questions about your health history. That questionnaire is then reviewed to see if we are able to offer what you need in a concierge medicine practice. After that review process you are able to schedule your initial consultation with Dr. Blossom.
There are times when it is not possible to be seen in person. However, in these cases telemedicine has been utilized well and our patients have been pleased with their access and treatment.
No. CCMD does not admit to any hospitals. We are a clinic based practice only. However, we are available for post hospitalization follow up and continuity of care.
If the problem is non urgent, call CCMD first and we will try to assist..However, call 911 if you have a life-threatening emergency. If you are out of the country you may of course call CCMD for guidance. Unfortunately in those situations we are unable to call in medications so coordinating prescriptions before travel is important.
